In our review, we highlight EGFL7. We summarize current knowledge on the molecular functions of EGFL7 and discuss its involvement in cancer, cardiovascular disorders, inflammatory conditions, and neurological diseases. We describe how EGFL7 regulates cell migration, vascular integrity, and immune cell infiltration, thereby shaping the vascular microenvironment and influencing disease progression. Furthermore, we review evidence linking altered EGFL7 expression to clinical outcomes and pathological processes across multiple disease settings. Particular emphasis is placed on emerging preclinical and clinical strategies aimed at therapeutically modulating EGFL7 activity. Our analysis highlights the considerable promise of EGFL7 as both a diagnostic biomarker and a therapeutic target, providing perspectives for future precision medicine approaches.
