Templates for withdrawing from an examination (elective and compulsory courses)

• Withdrawal due to illness during the examination must be reported immediately to the invigilator or the Examination Office by email. The form for the medical certificate is also required in the case of an interruption of the deadline for a Bachelor's, Master's or seminar paper, etc.
• In the case of a withdrawal with reason (§ 16 of the Examination Regulations), you must immediately submit this declaration of withdrawal (form) and proof of the reason for withdrawal to the Examination Office. In the case of withdrawal for reasons of illness, the application for withdrawal and the "Medical certificate" form must be submitted to the Examination Office within three working days.

You can inspect your written examinations and assessments of final papers at any time during ZIS office hours. However, please bring this completed and signed declaration for the inspection of written examinations with you.
