Geriatric patients and multimorbidity
Due to the demographic development in Germany, the proportion of geriatric patients in GP practices is continuously increasing. With advancing age, the number of treatment-relevant diagnoses, especially degenerative diseases, increases. A multimorbid patient suffers from two or more chronic diseases and is regularly treated by a general practitioner.

To date, there has been a lack of evidence-based recommendations for the diagnosis and treatment of patients with dizziness and/or balance disorders in primary care.
The "MobilE-PHY2" project is investigating whether an evidence-based care pathway - consisting of a decision aid for GPs and a decision tree for physiotherapists - can improve the mobility and participation of older patients with dizziness and/or balance disorders .
- Mobile- TRA - Mobility - the ability to move around outside your own four walls is a crucial prerequisite for independence, social participation and quality of life. However, illnesses often restrict mobility, especially in old age.
The aim of the MobilE-TRA project - "Care pathways and their impact on mobility, social participation and quality of life" is to investigate individual care pathways in elderly patients with dizziness or joint diseases.
