JOHANNA
The aim of the project “JOHANNA” is the analysis, design, piloting, and evaluation of a hybrid care counseling model to support self-determined aging in rural areas. By systematically combining digital and face-to-face formats, the project seeks to relieve family caregivers, strengthen informal care, and promote the social participation of older adults. JOHANNA follows an integrative approach that addresses both the improvement of counseling and support structures within the care context and the further development of digital counseling competencies and models in rural regions. Through the structured interaction of its work packages, the project establishes a scientifically grounded and transferable model of hybrid care counseling, enabling the development of sustainable, digitally supported care advisory structures in rural areas.
Funding
BMFTR funding measure “WIR! – Change through Innovation in the Region” within the programme “Innovation & Structural Transformation,” aimed at strengthening digital innovation and sustainable service structures in structurally disadvantaged regions.
The project aims to develop and scientifically validate a hybrid care counseling approach tailored to rural regions. By integrating digital and in-person formats, it seeks to strengthen support structures within the care context and promote self-determined aging.
Target groups comprise family caregivers, stakeholders within regional care and counseling structures in rural areas, and prospective Community Care Managers.
