JOHANNA

The aim of the JOHANNA project is to develop, pilot, and evaluate a hybrid Community Care approach that supports older people in living independently in rural areas. The project focuses on how Community Care Managers can provide low-threshold guidance and support, better connect existing support structures, and facilitate access to appropriate services. Particular emphasis is placed on the social and digital participation of older people and on supporting their access to digital information, communication, and support services. Based on needs identified through participatory methods, a role, task, and competency profile will be developed and translated into a modular, micro-credentialed qualification programme. The Community Care approach and the qualification programme will be piloted in practice and scientifically evaluated to develop transferable approaches for different rural regions and foster the sustainable networking of existing support structures.

Objective

Target group

The project addresses older people and family and informal carers, stakeholders from regional support, social and healthcare structures in rural areas, as well as prospective Community Care Managers.
