Overview of forms

Here you will find an overview of all forms relevant to your studies and examinations:

  • General forms (Form 05 BAföG, accident report for students)
  • Examination forms (application for additional module, declaration of waiver, medical certificate and application for withdrawal due to illness, application for registration and extension of final theses)
  • Degree program-related forms for examinations (degree programs Diplom Informatik, CMS, DSE)
  • Forms for the recognition of study and examination achievements
  • Examination law applications/documents (certificate application, information on the certificate of completion, certificate of achievement (80% certificate), clearance certificate)

Processing instructions:

  • Please use the form function of the PDFs.
    If the PDFs have drop-down menus, these may not work in older PDF readers and browser applications. Please download the file and use a current PDF reader to open it so that all form functions work.
  • All forms are writable PDFs. Please complete the forms digitally only. Forms filled out by hand must be rejected to avoid errors.
  • Forms must always be printed out and signed with an original signature. Digital signatures or PAD signatures are not legally binding signatures and are therefore not permitted and will be rejected by the Examination Office.
  • The printed and signed forms must be submitted by post or in person to the Examination Office for Computer Science. Forms submitted by e-mail will not be processed.

General forms

  • Certificate of benefit(Form 05 ) according to §48 BAföG and instructions from the BAföG office on how to apply
    Important: Please submit the original form 05 (printed and signed) to the Examination Office for Computer Science.
    Forms submitted by e-mail will not be accepted.

Recognition of study and examination achievements

Before submitting your application, please obtain information about important details and the credit transfer procedure or about credit transfer options from your responsible academic advisor!

Important! The application form must be completed in digital form using the PDF form functions!
Please submit the application only in paper form (printed and signed) by post or in person to the Examination Office for Computer Science!
Please ensure that the application and all documents to be submitted are complete. If the application or the attached documents are incomplete, it will not be possible to review and process them.

General application for credit transfer

  • Application for the recognition of study and examination achievements
  • The following documents must be submitted with the application for credit transfer of coursework and examinations:

    - Transcripts of records (in copy), transcripts of records (in copy), certificates in original or as officially certified copies (in German or English)
    - Module/course descriptions of the study and examination achievements applied for (in German or English)
    Note: If you only have links to the course descriptions, please only forward them to the Examination Office for Computer Science by e-mail after submitting the application and with reference to the application for credit transfer.
  • The certificate of credited semesters can only be confirmed after the credit transfer application has been processed

ERASMUS credit transfer application

  • Application for recognition of study and examination achievements to be completed as part of an ERASMUS study program (further information can be found on the ECTS creditability website)
  • Copies of the following documents must be attached to the application:
    - Transcript of Records,
    - Application for preliminary review of the recognition of study and examination achievements to be completed as part of an ERASMUS study program  confirmed by the ERASMUS coordinator,
    - Learning Agreement, and 
    - Copies of the respective module or course descriptions
    Note: If you only have links to the course descriptions, please only forward these to the Examination Office for Computer Science by email after submitting the application and with reference to the credit transfer application.

Credit transfer applications for internal change of degree program

Please note: In the case of an internal change of degree program, no module or course descriptions and no grade overviews or transcripts of records need to be enclosed. The completed application for credit transfer is sufficient.
The application for credit transfer must be submitted in paper form and completed in full to the Examination Office for Computer Science. If the application is incomplete, it cannot be reviewed and processed. Subsequent submissions or corrections by e-mail cannot be considered!

Examination law applications / documents

  • Application for the issue of certificate documents
  • Certificate of completion - issued by the Examination Office - application by ticket
    Important: Before the certificate of completion is issued, the application for the issue of the certificate documents must be submitted to the Examination Office for Computer Science
  • Certificate of achievement (80% certificate) - issued by the Examination Office - application by ticket
  • Clearance certificate - issued by the Examination Office - application by ticket
  • Additional module - application by ticket to the Examination Office with attached module description as a PDF attachment and stating the examiners
    Please note: Additional modules can only be applied for during ongoing studies that have not yet been completed and at the beginning of each semester.
    If the studies have already been completed, even if enrolment in the respective degree program continues until the end of the semester, it is no longer possible to apply for and take additional modules!
