Overview of forms

Here you will find an overview of all forms relevant to your studies and examinations:

  • general forms - form 05, form 10, accident report for students
  • Forms for examinations - declaration of waiver, form for certification of inability to take examinations and application for withdrawal due to illness, application for registration and extension of final theses
  • Degree program-related forms for examinations - degree programs Diplom Informatik, Master CMS, Master DSE

Processing instructions:

  • Please use the form function of the PDFs.
    If the PDFs have drop-down menus, these may not work in older PDF readers and browser applications. Please download the file and use a current PDF reader to open it so that all form functions work.
  • All forms are writable PDFs. Please complete the forms digitally only. Forms filled out by hand must be rejected to avoid errors.
  • Forms must always be printed out and signed with an original signature. Digital signatures or PAD signatures are not legally binding signatures and are therefore not permitted and will be rejected by the Examination Office.
  • The printed and signed forms must be submitted by post or in person to the Examination Office for Computer Science. Forms submitted by e-mail will not be processed.

General forms

  • Forms for the certificate of benefit Form 05 according to § 48 BAföG and Form 10 as well as instructions from the BAföG Office on how to apply can only be found on the BAföG Office website.

Important: Please submit the original Forms 05 and 10 (printed and signed) to the Examination Office for Computer Science.

Forms for examinations

Please note: Both forms (form for certification of inability to take the examination and application for withdrawal/extension due to illness) must besubmitted together in the originalno later than 5 working days after the examination date and can be sent by post or deposited in one of the TUD's official mailboxes/deadline mailboxes.
The postmark of the TU Dresden counts as the deadline.
Submission by e-mail is not permitted.

Please note: Both forms (form for certification of incapacity to take examinations and application for extension of the processing period for the thesis due to illness) must be submitted together in the originalwithout delay and can be sent by post or deposited in one of the TUD's official mailboxes/deadline mailboxes. The postmark of the TU Dresden counts as the deadline. Sending by e-mail is not permitted.

Please note: The sick days can only be recognized if they are reported within the processing time of the thesis for the postponement of the submission deadline and confirmed by the Examination Office in due time.

Recognition of study and examination achievements

Important! All credit transfer forms must be completed in digital form using the PDF form functions in legible font!

Please ensure that the application and all documents to be submitted are complete. If the application or the documents submitted are incomplete, it will not be possible to review and process them and the application will be rejected without processing.

Application for credit transfer of coursework and examinations

Application for transfer of credits for Computer Science students

  • Application for change of degree program
    (only applies to changes of degree program within the Faculty of Computer Science - for changes of degree program within TU Dresden, please use the application for transfer of study and examination credits)
  • Please print out the complete application and submit it with your original signature to the Computer Science Examination Office. In addition, please send it by ticket to the Credit Transfer Queue
  • Appendix 1 - Application for credit transfer for change of degree program - can be used if the application for change of degree program is not sufficient

Application for recognition of studies abroad (Erasmus)

  • Application for recognition of study and examination achievements completed as part of a study abroad program (further information can be found on the website Internationales | FAQ "How can I have credits earned abroad recognized?")
  • The following documents must be attached to the application as certified copies:
    - Transcript of Records
    - Application for preliminary examination confirmed by the ERASMUS coordinator for the recognition of study and examination achievements to be completed as part of an exchange program (Learning Agreement)
    - Copies of the respective module or course descriptions
    Note: If you only have links to the course descriptions, please forward them to the Examination Office for Computer Science by e-mail only after submitting the application and with reference to the credit transfer application.

Certificate application / examination law applications / documents

  • Application for the issue of certificate documents
  • Certificate of completion - issued by the Examination Office - application by ticket
    Important: Before the final documents are issued, the application for the issuance of the certificate documents must be submitted to the Examination Office for Computer Science
  • Certificate of achievement (80% certificate) - is issued by the Examination Office - application by ticket
  • Clearance certificate - issued by the Examination Office - application by ticket
  • Additional module - application with informal application (printed out and with original signature) and module description as an attachment and stating the examiners, to be submitted to the Examination Office for Computer Science
    Please note: Additional modules can only be applied for during ongoing studies that have not yet been completed and at the beginning of each semester.
    If the studies have already been completed, even if enrolment in the respective degree program continues until the end of the semester, it is no longer possible to apply for and take additional modules!
