Overview of forms

Here you will find an overview of all forms relevant to your studies and examinations:

  • General forms (Form 05 BAföG, accident report for students)
  • Examination forms (application for additional module, declaration of waiver, medical certificate and application for withdrawal due to illness, application for registration and extension of final theses)
  • Degree program-related forms for examinations (degree programs Diplom Informatik, CMS, DSE)
  • Forms for the recognition of study and examination achievements
  • Examination law applications/documents (certificate application, information on the certificate of completion, certificate of achievement (80% certificate), clearance certificate)

General forms

Forms for examinations

  • Waiver declaration (waiver of an examination attempt)
    Note: Please submit the original waiver form (printed and signed) to the Examination Office for Computer Science

  • Medical certificate and application for withdrawal from examinations due to illness
    Note: Processing can only take place if both documents are submitted in the original to the Examination Office for Computer Science. Documents submitted by e-mail will not be accepted!

  • Application for registration of the final thesis
    Note: If only digital signatures (signatures) are required, the application can be submitted by e-mail via

  • Application to extend the processing period for the thesis

Program-related forms

Recognition of study and examination achievements

Before submitting your application, please obtain information about important details and the credit transfer procedure or about credit transfer options from your responsible academic advisor!

  • Application for credit transfer of study and examination achievements
  • Supplementary sheet to the application for recognition of study and examination achievements
  • Please feel free to use the completion aid to save you and the Examination Office unnecessary frustration!
  • Application for preliminary examination for the recognition of study and examination achievements to be completed as part of an ERASMUS study program at TU Dresden (further information can be found on the website ECTS credit transferability)
  • The following documents must be submitted with the application for credit transfer:

    - Performance overviews, transcripts of records, certificates in the original or as officially certified copies (in German or English)
    - Module/course descriptions of the study and examination achievements applied for (in German or English)
    - Pre-filled form for the certificate of credited semesters

Important! The application form must be completed in digital form using the PDF form functions! Please submit the application both in digital form() and in paper form (printed and signed) to the Examination Office!

Examination law applications / documents

  • Application for the issue of certificate documents
  • Certificate of completion - issued by the Examination Office - application by e-mail
  • Certificate of achievement (80% certificate) - is issued by the Examination Office - application by e-mail
  • Clearance certificate - issued by the Examination Office - application by e-mail
  • Additional module - application by e-mail to the Examination Office with attached module description as a PDF attachment and stating the examiners
