Here you will find a thematic overview of all forms relevant to your studies and exams:

Instructions for use:
- Please use the form functions in the PDFs.
If the PDFs include dropdown menus, these may not function properly in older PDF readers or browser applications. Please download the file and use a current PDF reader to open it so that all form functions work. - All forms are fillable PDFs. Please fill out the forms digitally only. Handwritten forms must be rejected to avoid errors.
- Forms must always be printed out and signed with an original signature. Digital signatures or PAD signatures are not legally binding and are therefore not permitted; they will be rejected by the Examination Office.
- The printed and signed forms must be submitted by mail or in person to the Computer Science Examination Office. Forms submitted by email will not be processed.
General Forms
- Forms for the Certificate of Academic Achievement ( Form 05 pursuant to § 48 BAföG) and Form 10, as well as instructions from the BAföG Office regarding the application process, are available exclusively on the BAföG Office’s website.
Important: Please submit the original copiesof Form 05 and Form 10 (printed and signed) to the Computer Science Examination Office.
- Forms submitted via email will not be accepted.

Note: Both forms (Certificate of Inability to Take an Exam and Application for Withdrawal or Extension Due to Illness) must besubmitted together in original formno later than 5 business days after the exam date and may be mailed or deposited in one of the available official mailboxes ordeadline mailboxes at TUD.
The date of the postmark from TU Dresden is used to determine compliance with the deadline.
Submission via email is not permitted.

Form for certification of inability to take exams and application for an extension of the thesis completion period due to illness
Note: Both forms (Certificate of Inability to Take Exams and Application for an Extension of the Thesis Completion Period Due to Illness) must be submitted together in original formwithout delay and may be mailed or deposited in one of the available official mailboxes ordeadline mailbox at TUD. The date of receipt as stamped by TU Dresden determines compliance with the deadline. Submission via email is not permitted.
Note: Sick days can only be recognized if they are reported within the thesis writing period for the purpose of postponing the submission deadline and are confirmed by the Examination Office in a timely manner.

Credit Transfer for Coursework and Exams
Important! All credit transfer forms must be completed in digital form using the PDF form functions and in legible handwriting!
Please ensure that the application and all required documents are complete. If the application or the submitted documents are incomplete, review and processing are not possible, and the application will be returned unprocessed.

Application for Credit Transfer of Academic and Examination Achievements
- Application for the Recognition of Academic and Examination Credits
(applies to students transferring from other universities, ERASMUS, and
students changing degree programs within TU Dresden) - Please submit the application for the recognition of academic and examination achievements exclusively in paper form ( printed and with an original signature) by mail or in person at the Examination Office for Computer Science.
Applications submitted via the online ticket system will not be processed. - Appendix 1 - Application for Recognition of Academic and Examination Credits
- Once the examiners have signed the form, please print out Appendix 1 and submit it as a PDF along with the corresponding module descriptions and a certified transcript of records (Transcript of Records), as well as the Learning Agreement for ERASMUS and, if applicable, an application for preliminary review via the Ticket system.
- Certification of credited semesters can only be confirmed after the credit transfer application has been processed
Credit Transfer Application for Students Changing to the Computer Science Program
- Credit Transfer Application for Change of Major
(applies only to program changes within the Faculty of Computer Science—for program changes within TU Dresden, please use the Application for Credit Transfer of Academic and Examination Achievements) - Please print out the complete application and submit it with an original signature to the Computer Science Examination Office. Additionally, please send it via ticket to the Credit Transfer Queue
- Appendix 1 – Credit Transfer Application for Change of Major – may be used if the application for change of major is insufficient
Transcript Request / Examination-Related Applications / Documents
- Application for the Issuance of Transcript Documents
- Certificate of Completion – issued by the Examination Office – request via ticket
Important: Before degree documents can be issued, the application for the issuance of degree documents must be on file with the Examination Office for Computer Science - Certificate of Academic Progress (80% Certificate) – issued by the Examination Office – request via ticket
- Certificate of No Objection – issued by the Examination Office – request via ticket
- Additional module – Application via an informal request (printed and with an original signature) and module description as an attachment, specifying the examiners; to be submitted to the Computer Science Examination Office
Note: Additional modules can only be applied for during the current, not yet completed degree program and at the beginning of each semester.
If the degree program has already been completed—even if enrollment in the respective degree program continues until the end of the semester—it is no longer possible to apply for or take additional modules!
